BIPOC USHR community navigators provide outreach and follow-up services addressing the social and emotional needs of BIPOC individuals on PEI through assessment of needs, support, connecting to resources, advocacy, system navigation, identification of system gaps and engaging community to fill those gaps.
Our community navigators respond to vulnerable BIPOC individuals in need of navigation support and advocacy around issues of justice, education, health care, housing, employment, discrimination, and other needs as they arise.
Community navigators work to provide information about services, provide practical supports, assist in filling out forms, accompany in meetings, reach out to their networks, and provide advocacy in other ways as necessary.
